Job summary

Care Initiatives Hospice in Cedar Rapids is seeking an experienced Hospice Clinical Manager - RN to lead our clinical team. You will oversee visits, ensure quality care, and coordinate care from referral to end of life, all while maintaining compliance with guidelines.

The role requires an active RN license, CPR certification, hospice experience, and leadership background. CHPN is preferred or obtainable within 24 months, with a strong focus on compassionate, patient-centered care.

Qualifications

• Active RN license in the state of operation.
• Active CPR certification.
• Hospice or palliative care experience.
• At least one year in a leadership or charge nurse role.
• Two years of recent clinical hospice or related nursing experience preferred.
• CHPN preferred or able to obtain within 24 months.

Responsibilities

• Oversee visits from referral-to-admission through death processes.
• Ensure quality care, documentation and collaboration meet guidelines.
• Coordinate direct care, ensure hospice eligibility, and oversee records.
• Provide orientation, education, training and competencies as needed.
• Maintain and implement policies to meet patient and family needs under regulations.
• Support and facilitate educational directives from administration.

Care Initiatives Hospice is looking for an experienced Hospice Clinical Manager - RN to support and lead our Cedar Rapids hospice clinical team. Care Initiatives, created in 1989, is the largest non-profit single operator of quality nursing homes in Iowa. Key Responsibilities Of a Hospice Clinical Manager

• Oversees all visits from referral-to-admission and through death processes.
• Ensures quality care, documentation and collaboration are within guidelines.
• Supports direct care coordination, ensures hospice eligibility, and oversees recordkeeping functions.
• Provides orientation, education, training and/or competencies, as needed.
• Maintain and implement operational policies and procedures to meet patient and family needs in compliance with federal, state, and local requirements guidelines and Care Initiatives Hospice policies and procedures.
• Support and facilitate educational directives of the Administrator and/or corporate leadership.

What it takes to be a Clinical Manager with Care Initiatives Hospice

• Active RN license (or ability to activate) in the state(s) in which you operate.
• Active CPR certification.
• Hospice or palliative care experience.
• At least one year in a leadership or charge nurse position or relative experience.
• Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.
• Home access to the internet.
• Hospice and Palliative Registered Nurse Certification (CHPN) preferred, or ability to obtain within 24 months of employment.
