Job summary

AccentCare is seeking a Hospice Clinical Liaison RN to support patient care coordination across Atlanta, GA and Gwinnett County. This full-time position follows a M-F schedule with limited on-call requirements.

You will liaise with admissions, care teams, and providers to promote hospice services and ensure care excellence. You should bring at least one year of RN experience in the local market, strong knowledge of home health regulations, and a background in medical sales if possible.

Qualifications

• One year of RN experience in local market with knowledge of governmental home health regulations and care plan development techniques.
• Previous medical sales experience preferred.
• Licensed RN currently licensed in the state of operation.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Work with Hospice Care Consultants to develop and implement individualized marketing plans.
• Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
• Act as a liaison with AccentCare Hospice Medical Director, Team Physicians and Hospice Physicians for providers and patients/families.
• Make inpatient visits at contracted facilities and communicate with Admissions Department, Team Directors and/or Primary Nurse.
• Provide consultation on hospice and palliative care.

Be the Best Clinical Liaison RN You Can Be

If you meet these qualifications, we would love to meet you:
