Paradigm Health is seeking a full-time Hospice Clinical Coordinator to deliver compassionate care to patients and families across a local Indiana territory. The role includes overseeing RNCMs, coordinating with interdisciplinary teams, and ensuring high-quality hospice services while maintaining compliant operations.

Qualifications

• Active RN license in Indiana required.
• 2 years hospice experience required.
• Knowledge of hospice regulations and COPs.
• Valid driver’s license and reliable vehicle.
• CPR certification.
• Proof of current automobile insurance.
• Health certificate and TB testing.
• Criminal background check.

Responsibilities

• Oversee day-to-day clinical operations for RNCMs and on-call nursing staff.
• Maintain and analyze clinical logs (falls, infections, incidents) and participate in QAPI.
• Ensure timely completion of HIS measures and act as HIS administrator.
• Manage medication, DME, and supply utilization for hospice patients.
• Participate in monthly hospice pharmacy calls and coordinate with pharmacy reps.
• Support care plan meetings and difficult patient care conversations.
• Handle grievance follow-ups and service recovery for patients.

Job Description

• Schedule: Monday- Friday 8:00 am - 5:00 pm
• Location: In office of posting city, daily
• Hospice coordinator for up to 8-10 RNCMs (target range but could vary) as well as other hospice nurses (i.e. on-call nursing) and aides. Develops relationships with partnering disciplines (i.e. Supportive Services) and serves as a resource and offers support/guidance when indicated.
• Works in conjunction with other Clinical Coordinators to ensure consistent messaging and education for all Paradigm Health hospice clinicians.
• Oversees day-to-day clinical operations for associated RNCMs (including but not limited to morning call, questions from the field, visit coverage for staff call-ins and/or patient PRN visit needs).
• Maintains clinical logs (i.e. falls, infections, incidents) for applicable patients and brings data to QAPI. Identifies trends and executes improvement plans.
• Completes grievance follow-up promptly for associated patients to ensure service recovery. Documents all follow-up and re-education efforts.
• Oversees medication, DME, and supply utilization for associated patients to ensure patient needs are met while remaining fiscally responsible.
• Participates with monthly hospice pharmacy call/review and addresses suggested changes with team as indicated. Brings pertinent questions/concerns from field staff to meeting to discuss with pharmacy reps.
• Supports RNCMs and associated hospice care team with care plan meetings or challenging patient care conversations/situations when indicated.
• Oversees timely and accurate completion of HIS measures and acts as the HIS Administrator for applicable patients.

Requirements

• Active RN License in the state of Indiana
• 2 years Hospice Experience required
• Knowledge of hospice regulations and COPs
• Valid state driver's license and reliable automobile.
• CPR Certification
• Proof of current automobile insurance.
• Current health certificate/physical examination and TB testing.
• Criminal background check.
• Currently CHPN (Certified Hospice and Palliative Nurse) or willingness to obtain certification within first year of beginning new position as Clinical Coordinator.
