Job summary

Transcend Hospice (2) is seeking a compassionate Certified Home Health Aide (CHHA) to provide personal care services for patients nearing end of life. You will work under the Director of Nursing and Aides Manager, document care electronically within 24 hours, and support families in bereavement within the IDG care plan guidelines.

The ideal candidate has at least 2 years of CHHA experience, a high school diploma or GED, and a valid driver's license with reliable transportation.

Qualifications

• Must have a High school diploma or GED.
• Graduate from an approved training and competency course.
• Minimum 2 years of experience as a CHHA preferred.

Responsibilities

• Provide quality nursing care to residents.
• Accept close, direct supervision by Primary RN and RN Supervisor.
• Observe vital signs as directed.
• Bathing - sponge, shower or tub.
• Shampoo or care of hair; no tinting.
• Shaving patients.
• Oral hygiene including dentures care.
• Nail/foot care, diabetic care as directed by RN.

Job description

Looking for a dedicated, compassionate CHHA who will provide personal care services for our patients preparing for end-of-life. You will work under the direction of the Director of Nursing and Aides Manager. Keep basic computer documentation of personal care services rendered and other appropriate services on a daily. Patient documentation will be completed electronically within 24 hours of the visit. The qualified candidate will provide assistance and understanding to the family in time of bereavement. As a CHHA, you will work as a member of the Hospice team in providing Hospice care within the guidelines of the IDG care plan.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide quality nursing care to residents in an environment which promotes their rights, dignity, freedom of choice, and their individuality;
• Accept close, direct supervision by Primary RN and RN Supervisor
• Observe temperature, pulse, respirations, blood pressure and weight of patients when so directed;
• Bathing - sponge, shower or tub;
• Shampoo or care of hair; does not include hair coloring or tinting;
• Shaving patients;
• Oral hygiene including care of dentures;
• Nail/foot care, diabetic care as directed by RN;

Must have High school diploma or GED

Graduate from an approved training and competency course.

Minimum 2 years of experience as a CHHA preferred.

Active patient contact within past two years, preferred.

Driver's License and Reliable Transportation required.
