Job summary

Alternate Solutions Health Network is seeking a Hospice Chaplain to deliver spiritual care to patients and families in our hospice program. The role works within an interdisciplinary team to address identified spiritual needs and coordinates care with other chaplains and counselors.

The position emphasizes compassionate communication, flexibility, and sensitivity to diverse beliefs. A seminary degree is preferred, and hospice experience is a plus; a valid driver’s license is required.

Qualifications

• Graduate of accredited seminary or school of theology; BD/MDiv or equivalent preferred.
• Experience with death and dying; hospice philosophy knowledge.
• Hospice experience preferred; valid driver license and auto insurance.

Responsibilities

• Provide direct spiritual care to patients, families and caregivers within hospice.
• Coordinate care with interdisciplinary team and spiritual counselors.
• Maintain records of spiritual care services and support internal education.
• Arrange funeral or memorial services as requested and support community clergy.

Eligible teammates will also receive paid time off, opportunity to participate in 401k, company paid life insurance and access to a robust Employee Assistance Program.HOW YOU'LL WORK:You'll provide direct spiritual care to patients, families and caregivers.MAJOR AREAS OF RESPONSIBILITY:Spiritual Care: Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner. Facilitate the development in the interdisciplinary group plan of care to meet identified spiritual needs.Administrative: Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. Documents direct services and ongoing communication with community chaplains and spiritual counselors.Customer Care: Provide for funeral or memorial services for patients as requested. Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.Teamwork: Provide consultation, education, and support to the interdisciplinary group on spiritual care.
