Job summary

Gentiva is seeking a compassionate Hospice Chaplain for part-time, weekdays dayshift to provide non-denominational spiritual and emotional support to patients and families at the end of life. You will collaborate with an interdisciplinary team, assess needs, and develop individualized care plans.

You will conduct visits, prayers, and rituals, and assist families during grief, offering bereavement support while maintaining clear documentation of interventions and outcomes.

Qualifications

• Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology.
• At least one ACPE-accredited CPE unit.
• Degrees from accredited institutions preferred; AVPO approval for non-accredited.

Responsibilities

• Provide spiritual and emotional support to patients and families.
• Offer non-denominational, inclusive care across faiths.
• Participate in patient care conferences with the interdisciplinary team.
• Develop individualized spiritual care plans.
• Conduct visits, prayers, rituals as requested.
• Assist families during grief, loss, or transition; provide bereavement support.
• Build relationships with local faith leaders and community resources.
• Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
