Job summary

Crossbridge Hospice, LLC is seeking a Hospice Chaplain to provide spiritual and bereavement support within an interdisciplinary team, shaping care plans that honor each patient’s journey. The role emphasizes compassionate care and collaborative decision‑making with patients, families, and staff.

The position requires a Bachelor’s degree in a related field and experience in grief counseling, with preference for a Master’s degree and CPE.

Qualifications

• Bachelor’s degree in religious studies, social work, psychology, counseling, pastoral education, or a related field.
• Valid driver’s license and good driving record.
• Experience in grief and loss counseling.
• Availability during agency operating hours and as needs arise.
• Strong written and verbal communication skills.
• Demonstrated leadership, organization, and time‑management skills.
• Proficiency with computers, including email, clinical software, word processing, and spreadsheets.

Responsibilities

• Provide spiritual and bereavement counseling to hospice patients and their families, and assist them in identifying and addressing their spiritual and grief‑related needs.
• Actively participate in interdisciplinary group (IDG) meetings and maintain open communication with all team members.
• Uphold strict patient confidentiality and ethical standards in all interactions.
• Facilitate connections to community spiritual or faith‑based resources when appropriate.
• Preside over funerals or memorial services at the request of the family.
• Support Age‑Friendly Care by incorporating the 4Ms—What Matters, Medications, Mentation, and Mobility—into spiritual care approaches.
