JourneyCare Hospice is seeking a Hospice Chaplain to join our team. The role includes providing spiritual care, bereavement counseling, and coordinating with an interdisciplinary team in a compassionate hospice setting in Illinois.

Qualified candidates will have seminary training, interfaith sensitivity, and a valid driver’s license; CPE is preferred. Hours are Monday–Friday with on-call expectations, serving Libertyville, Gurnee, Mundelein, and surrounding areas.

Qualificações

• Graduate of an accredited seminary or theology school; CPE preferred
• Hospice experience with death and dying strongly preferred
• Interfaith setting comfort; respect diverse beliefs and lifestyles
• Valid driver’s license and proof of insurance

Responsabilidades

• Performs spiritual bereavement assessments for patients and families in a timely manner and integrates needs into the care plan
• Provides direct spiritual care and bereavement counseling to patients and families
• Delivers exceptional service to patients, families, referral sources and coworkers; supports team as needed
• Documents patient services accurately and promptly to reflect the plan of care
• Attends group meetings to coordinate care and interdisciplinary discussion
• Provides consultation and education on spiritual care and bereavement to the interdisciplinary team
• Participates in Continuous Care and on call, if necessary
• Supervises spiritual care volunteers as needed with patient families/caregivers
• Directs the development and implementation of bereavement plan and services through one year after patient death
