Job summary

Pathwell Health seeks a dedicated Certified Nursing Assistant to support patients at home or in a care facility under an LPN care plan.

You will provide routine healthcare, assist with activities of daily living, monitor changes in health, and maintain accurate documentation while upholding confidentiality. Strong communication and organizational skills are essential to coordinate care and ensure comfort and safety for clients.

Qualifications

• One year of caregiving experience (preferred)
• Valid Virginia CNA license

Responsibilities

• Provide companionship and socialization
• Escort clients to recreational activities and medical appointments
• Perform light housekeeping duties, as needed, to maintain a safe environment (i.e. changing bed linens, laundry, dishes, etc.)
• Help clients with personal care needs (i.e. Home duties, cooking, light cleaning, medication reminders, etc.)
• Assist clients with Activities of Daily Living (ADL’s)
• Perform light housekeeping duties, as needed, to maintain a safe environment (i.e. changing bed linens, laundry, dishes, etc.)
• Teach and encourage self-care skills
• Maintain complete and accurate documentation of clients’ care
• Maintain confidentiality in all matters related to the clients’s care
• Perform other duties as assigned

Job description

The Certified Nursing Assistant will provide routine individualized healthcare based on a care plan set by LPN to the hopsice elderly, convalescents, or persons with disabilities at the patient's home or in a care facility. Monitor or report changes in health status. May also provide personal care such as bathing, dressing, and grooming of patient.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

• Provide companionship and socialization
