Job summary

Gentiva is seeking a dedicated Hospice Patient Care Manager to oversee clinical operations, coordinate care, and mentor staff. The role reports to the Executive Director or Administrator and requires strong leadership to ensure compliant, high-quality hospice services.

You will direct patient care, triage referrals, and oversee IDG meetings. The ideal candidate holds an RN license with a BSN preferred, plus 3+ years in direct patient care including hospice/home care, and a track record of

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care, including 2 years in hospice/home health.
• Demonstrated knowledge of hospice principles, regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Coordinate visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor and train associates while ensuring compliance.
• Ensure care delivery meets federal/state/local standards and policies.
• Support performance improvement and operational efficiency at your location.
