A hospice care provider based in New York is seeking a Registered Nurse, RN, to join their interdisciplinary team. The Registered Nurse will provide patient-centered care in accordance with hospice practices, including patient assessments and education. The ideal candidate will have a valid nursing license, at least two years of experience in hospice or community health, and will be skilled in communication and case management. This role requires reliable transport and possibly on-call responsibilities.

Competenze

• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Currently licensed to practice nursing in the state.

Mansioni

• Coordinate interdisciplinary plan of care.
• Perform supervision of nursing tasks delegated to other staff.
• Document patient assessments and care at the time of the event.
• Participate in quality improvement programs.
• Provide education and training for patients and families.
