Bristol Hospice is seeking a Care Consultant (Hospice Liaison) to build strong relationships with referral sources and serve patients and families with compassionate, patient-focused end-of-life care. You will travel across the Bristol Hospice service area, presenting to providers and facilities while coordinating with clinical teams.

Qualifications

• Able to communicate with the general public and represent Bristol positively.
• Able to articulate Hospice Conditions of Participation and transitions in care.
• Experience in sales, marketing, or education/training.
• Flexible scheduling including some evenings and weekends.

Responsibilities

• Build relationships with referral sources in facilities, hospitals, physician offices, and community partners.
• Be out in the community daily, representing Bristol Hospice on the ground.
• Make outreach calls and deliver informative presentations to referral sources.
• Collaborate with clinical and operational teams to ensure seamless care.
• Meet with community partners about hospice services for their patients.
• Participate in regular team calls to coordinate efforts.
• Drive growth by advocating Bristol Hospice to referral sources.
