DayToDay Health is seeking a physician to provide hospice medical oversight, clinical consultation, and direct patient care in collaboration with an interdisciplinary healthcare team in Hartford, CT.

You will certify eligibility, guide pain and symptom management, review plans and records, and conduct home visits to support comprehensive end-of-life care with the care team.

• Proficiency in patient assessment, pain and symptom management, and care plan evaluation.
