Job summary

The Cleveland Clinic Hospice team is seeking a Night-Shift Home Health Aide to support patients in their homes, at the Hospice Care Center, or other locations within the Southern Territory. You will provide compassionate, patient- centered care and collaborate with nurses to maintain comfort and dignity.

This role requires an STNA, Ohio driver’s license, and BLS, with at least one year of clinical experience.

Qualifications

• High School Diploma or GED required.
• STNA certification in Ohio.
• One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting.
• Valid Ohio driver’s license and proof of $100,000/$300,000 automobile insurance coverage.
• Basic Life Support (BLS) certification within orientation period.

Responsibilities

• Perform basic patient care activities.
• Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.
• Assist patients with ADLs and transfers/transport.
• Report and document abnormal assessment findings or changes to nursing staff.
• Keep patient environment clean and orderly.
• Maintain up-to-date documentation regarding patient condition and care.
• Escalate changes in patient condition to physician as appropriate.

A caregiver in this position works nights, 9:00pm - 5:00am. This would be 3 days a week, 8 hour shifts. Every 3rd weekend at the Hospice Care Center. This role will support our Southern Territory and the Hospice Care Center.

A caregiver who excels in this role will:

• Perform basic patient care activities.
• Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.
• Assist patients with tending to personal care, activities of daily living and transfers/transport.
• Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.
• Assist with keeping the patient's environment clean and orderly.
• Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
• Escalate change in patient condition to patient’s physician as appropriate.

Minimum qualifications for the ideal future caregiver include:

• High School Diploma or GED
• State Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse’s Aide Registry
• One year of clinical experience as an Aide in an acute, rehab or skilled nursing setting
• Valid Driver’s License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) within orientation period

Preferred qualifications for the ideal future caregiver include:

• Hospice and Palliative Care Nurse Assistant (CHPNA) Certification
• Home Care experience

Physical Requirements:

• Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.
• For field staff only: Able to drive to patients’ homes and/or facilities.
• Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
• Physical Demand requirements are in excess of those for Light Work.

Personal Protective Equipment:

• Follows standard precautions using personal protective equipment as required.

Pay Range

Minimum hourly: $18.25

Maximum hourly: $24.95
