Job summary

Longleaf Hospice and Palliative Care is seeking a Hospice Aide to deliver compassionate in-home care in the Roswell area. You will assist with personal care, mobility, meals, and daily activities, while reporting patient status to a supervising nurse. A valid driver’s license and reliable vehicle are required.

Successful completion of a state-registered training program and current CPR are necessary. The role emphasizes safety, documentation, and patient comfort within the community we serve.

Qualifications

• Requires completion of a state-approved training program and competency evaluation or licensure.
• Good verbal and written communication and problem solving skills.
• Valid driver’s license, reliable insured vehicle for home visits, CPR current.

Responsibilities

• Provide personal care including baths, grooming, and basic hygiene.
• Assist with dressing, meals, and daily activities as assigned.
• Monitor and report changes to supervising nurse or therapist.
• Ensure patient safety and comfort, including skin care and ROM exercises as ordered.
• Maintain clean, safe living environment and proper documentation.

As a Hospice Aide, you will perform various services for patients as necessary to meet the patient’s personal needs and to promote comfort through:

• Performing personal care needs after given a patient's assignment and attending to his/her requests promptly
• Meeting safety needs of patient and using equipment safely and properly
• Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens as often as assigned
• Assisting in dressing and undressing patients as assigned
• Assisting with patient meals as assigned
• Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence
• Reporting on patient's condition and significant changes to the assigned supervisory nurse
• Assisting in ambulation and exercise as instructed by the primary care nurse or therapist
• Offering and assisting with bedpans and urinals
• Providing assistance as assigned with light laundry needs
• Performing range of motion and other simple procedures as an extensional therapy service as ordered with appropriate completed/demonstrated skills competency
• Providing respite for patient's family/caregiver when on‑site, as appropriate
• Keeping patient's living area clean and orderly, as assigned
• Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct
• Respectful of patient/family/caregiver environment and patient's personal needs

Requirements

• Successful completion of a state established or other training program that meets the requirements of S 484.36(a) and a competency evaluation program, or state licensure program that meets the requirements of S 484.36(b), or a competency evaluation program or state licensure program that meets the requirements of 42 CFR § 484.36(b)
• Good verbal and written communication and problem solving skills
• Responsible valid driver’s license; reliable, insured, automobile for making home visits required
