Elara Caring in Oklahoma is seeking a Hospice Certified Nurse Assistant/Hospice Aide to provide skilled care under the Plan of Care and RN guidance. You will monitor patients, educate families, and document medications while supporting safety and independence in home settings.

The role requires a High School Diploma or GED, 1 year of hospice/aide experience, and state training/competency; valid driver’s license or reliable transportation is needed for travel between patient homes.

• Help prevent hospitalizations through proactive patient care.
• Maintain compassionate communication with patients, families, and care team.

Job Description

As a Hospice C.N.A /H.H.A., you’ll contribute to our success in the following ways:

• Provide skilled nursing care according to the individualized Plan of Care, physician orders, and direction of the RN.
• Monitor patient condition and promptly report changes, concerns, or safety issues to the RN Case Manager, physician, and care team.
• Collaborate with the RN Case Manager to address patient needs and recommend updates to the Plan of Care when appropriate.
• Educate patients and caregivers on self-care techniques to support safety, independence, and overall health.
• Document medication regimens and maintain accurate, up-to-date medication profiles.
• Help prevent avoidable hospitalizations and emergency room visits through proactive, coordinated patient care.
• Provide compassionate, high-quality care while maintaining positive communication with patients, families, physicians, and the care team.

What is Required?

• High School Diploma or GED equivalent required
• 1 year of experience as a Hospice Aide or Nursing Assistant is required
• State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a home health environment is preferred
