Choice Hospice, Inc. is seeking a dedicated Hospice Aide to provide compassionate care to patients in their homes or residence, under RN supervision, following the Care Plan. You will support safe transfers, assist with daily living activities, report changes, and help ensure a safe, clean environment.

This full-time role emphasizes infection control, privacy, and respectful interaction with patients and families. It offers collaborative teamwork and meaningful end-of-life care opportunities.

Qualifications

• Direct patient care under RN direction and according to the Plan of Care.
• Safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity of care.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patients to ensure continuity of care.
• Practice infection control principles to provide a clean, safe and comfortable environment.
• Promote respectful communication to patient/family and other employees.
• Provide an environment which promotes privacy and respect for patients and property.
• Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
• Contribute to the management and efficient operation of the agency and demonstrate time management.
• Provide skills necessary to perform treatments and procedures according to agency policy.
• Promote the agency philosophy and administrative policies to ensure quality of care.

Position Summary

The Choice Hospice Aide is responsible for providing care to the patients including assistance with personal healthcare and related services to the patient in their place of residence and to assist in providing a safe and clean environment, The Hospice Aide will work cooperatively with patient and family and share observations and problems with the supervisor. We love our Hospice Aides, as they are a key part of the care team who help us deliver on the mission of hospice.

As a Hospice Aide , you will:

Requirements

• Current CNA/HHA & LTC certifications
• Valid Driver’s License
• Valid Auto Insurance
• CPR Certification
• HomeCare HomeBase experience preferred
