Job summary

ExcelaCare is seeking a compassionate Hospice Aide to join our team providing direct patient care in the patient's home under the direction of a registered nurse. You will assist with transfers, mobility, nutrition, and daily living activities while maintaining a safe, clean, and respectful environment.

The role requires completion of a Nurse Aide program or state-approved competency evaluation, a valid driver’s license, and the ability to travel to patient homes.

Qualifications

  • Education: High School Diploma preferred.
  • Experience: At least one year preferred as a hospice aide or nursing assistant.
  • License / Certification: Current driver’s license and proof of auto liability
  • Credentialing: Nurse Aide training/competency eval program approved by state

Responsibilities

  • Provide direct patient care under the direction of the RN and according to the Aide Plan of Care.
  • Assist in transfers and ambulation per agency policy.
  • Report changes and document care rendered to patient to ensure continuity of care.
  • Maintain infection control and provide a clean, safe environment.
  • Communicate respectfully with patient/family and other staff.
  • Support privacy and safe handling of patient information.
  • Assist with nutrition and fluid intake as needed.
  • Support agency operations and time management.
  • Assist with treatments per agency policy.
  • Uphold agency philosophy and policies to ensure quality care.

Summary:

The primary function is to provide healthcare and related services to the patient in their place of residence; assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

Essential Functions:

  • Provide direct patient care under the direction of the RN and according to the Aide Plan of Care.
  • Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
  • Provide necessary skills to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
  • Practice accepted infection control principles. Provide a clean, safe, and comfortable environment.
  • Promote positive, supportive, respectful communication with patient/family and other employees.
  • Provide an environment which promotes respect for patients, privacy, and property.
  • Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
  • Contribute to the management and efficient operation of the agency and demonstrate effective time management skills.
  • Provide skills necessary to perform treatments and procedures according to agency policy.
  • Promote the agency philosophy and administrative policies to ensure quality of care.

Additional Responsibilities:

  • Performs related duties as assigned or requested.
  • Conforms to all applicable Agency policies and procedures.
  • Participates actively in continuing education and in-services.
  • Maintains confidentiality of patient information and business trade practices
  • Assumes accountability for reporting incidents and complaints according to Agency policy.

Knowledge/ Skills/ Abilities:

  • Must pass Aide Competency Evaluation Skills Checklist that meets §418.76(c) and written skills test.

Age-Related Competencies:

Demonstrates the basic knowledge and skills necessary to identify age-specific patient needs appropriate for this position.

Treats all information and data within the scope of the position with appropriate confidentiality and security

  • Cooperates fully in all risk management activities and investigations.
  • Keeps abreast of changes in health care law.
  • Maintains agency/program compliance with local, state, and federal laws as well as state accreditation standards.

Minimum Position Qualifications:

  • Education: High School Diploma preferred.
  • Has successfully completed one of the following:

A) Training program/competency eval as specified in §418.76 (b) (c) of the Medicare regulations

B) Competency eval program that meets §418.76 (c)

C) Nurse Aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry

D) State Licensure program that meets requirements of §418.76 (b)(c)

  • Experience: At least one year preferred, as a hospice aide or nursing assistant in a hospital, nursing home, hospice agency, hospice, or long-term care facility or completion of Certified Nursing assistant or Certified Home Health Aide Program.
  • Training: Appropriate competency assessments completed
  • License / Certification: Current driver’s license and proof of auto liability

Environmental Conditions:

Works in patient's home in various conditions, possible exposure to blood and body fluids and infectious diseases. Ability to work flexible schedule, ability to travel locally, some exposure to unpleasant weather. Moderate noise level; tasks may involve moderate stress and emotional demands.

Physical Requirements:

Prolonged standing and walking required. Ability to move up to 50 lbs. and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions and resource acquisition; meet patient/family psychosocial needs. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.
