Providence at Home with Compassus is seeking a Hospice Registered Nurse to deliver compassionate, patient-centered care in patients' homes across Thurston, Mason, and Lewis Counties.

You will assess needs, develop plans with the Interdisciplinary Group, administer medications, and participate in on-call rotations, while upholding dignity at end-of-life. This role offers a sign-on bonus and comprehensive benefits.

Qualificações

• Associate or Bachelor's degree in Nursing required.
• Current, unencumbered RN license in the state of employment.
• Within 30 days of hire: BLS certification (American Heart Association).
• Preferred: 1 year hospice RN experience.

Responsabilidades

• Complete assessment of patient and family to determine Hospice needs.
• Provide a complete physical assessment and nursing care using the nursing process.
• Initiate and revise the Plan of Care in collaboration with the IDG.
• Administer medications and treatments as prescribed by the IDG Plan of Care.

We are currently seeking a Hospice Registered Nurse (RN) who is passionate about delivering person-centered care in the home environment. The Registered Nurse plans, organizes and directs Hospice care and is experienced in nursing, with an emphasis on community health education / experience.The Hospice nurse builds from the resources of the Interdisciplinary Group (IDG) to plan and direct services to meet the needs of individuals and families within their home environments. May provide patient care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.Position Specific ResponsibilitiesCompletes assessment of patient and family to determine Hospice needs.Provides a complete physical assessment and history and provides professional nursing care by utilizing all elements of the nursing process.Writing and initiating Plan of Care in collaboration with the IDG.Regularly reevaluating patient/family/caregiver needs and revising the Plan of Care as necessary.Initiates the Plan of Care and makes necessary revisions as patient status and needs change.Develops a care plan in conjunction with the patient/family/caregiver and IDG which establishes goals based on nursing diagnosis and incorporates palliative nursing actions.May administer medications and treatments as prescribed by the IDG approved Plan of Care.Participates in on-call duties as defined by the on-call procedures.Maintains productivity standards as set by organization guidelines.Documentation/Information Management:Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.Supervises ancillary personnel and delegates responsibilities when required.Participates in orientation, education, and development of team members.Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.Education and/or ExperienceRequired - Associate or Bachelor's degree from an accredited School of Nursing.Preferred - 1 year experience practicing as a registered nurse. Experience as a Hospice CNA or Hospice LPN will be considered.Certifications, Licenses, and RegistrationsRequired - Current, unencumbered (State of Employment) License as a Registered Nurse.Required – Within 30 days of hire - National Provider BLS - American Heart Association (Vendor Managed).Physical Demands and Work Environment:The demands of this role necessitate a team member to effectively perform essential functions.
