Job summary

CovenantHealth at Home with Compassus, Hospice of Lubbock is seeking an RN intake coordinator to manage referrals, admissions, and initial care plans for hospice patients in the Lubbock area. This role ensures accurate patient records, orders, and collaboration with physicians and social workers.

Responsibilities include coordinating services, leading team conferences, and providing clinical expertise while supporting minimal supervision.

Qualifications

• Requires Texas Registered Nurse License and current clinical licensure.
• Hospice experience and nursing leadership preferred.
• Education: BSN preferred; hospice and bereavement trainings required.

Responsibilities

• Coordinate admissions for Hospice of Lubbock patients, including patient information, orders, and plans of care.
• Assist with team conferences and conduct on-call coverage.
• Oversee assignment of patients to interdisciplinary team and ensure accurate patient records.
• Coordinate services, equipment, supplies, and prescriptions for new admissions.
• Supervise intake LVN and assist with policy and procedure updates as needed.
