Job summary

VITAS Healthcare is seeking an Admissions RN in Florida who will present the company philosophy and services to patients and families to secure appropriate admissions. The role involves information gathering, education, and coordination across the admission process, with emphasis on evaluating eligibility and building strong referral relationships.

The RN must be licensed in the state, possess strong presentation and communication skills, and be able to navigate the VITAS information system

Qualifications

• Licensed RN in the state where duties are performed.
• Experience in hospital, physician practice, LTC or ALF preferred.
• Strong presentation skills and ability to educate families.
• Knowledge of intake, admission and transition to care processes.
• Ability to use VITAS information system and mobile platforms.

Responsibilities

• Present VITAS services and philosophy to patients and families to aid admission.
• Participate in information gathering, analysis, education and form completion.
• Evaluate patients for hospice appropriateness and admit when criteria are met.
• Obtain agreement for admission of referred patients meeting criteria.
• Build effective referral source relationships to support quality care.
• Maintain clinical and professional competence and internal relationships.
• Educate patients/families about hospice and palliative care.
