Job summary

Addus Family Company is seeking a compassionate Hospice Admissions RN to coordinate and complete hospice admissions for patients in their homes. You will conduct initial assessments, verify eligibility, obtain consents, and create individualized plans of care with physicians and the interdisciplinary team.

You will educate patients and families on hospice philosophy, services, and symptom management, ensuring a smooth transition of care.

Qualifications

• Active RN license in good standing.
• Experience in hospice or home-based care preferred.
• Ability to educate patients and families about hospice services.
• Strong communication and collaboration with physicians and care team.

Responsibilities

• Coordinate and complete hospice admissions for patients in their homes.
