VITAS Healthcare is seeking a Hospice Admissions Nurse in Tallahassee, FL. You will bridge hospital or community care to hospice, guiding patients and families through a pivotal transition with empathy and clarity.

As part of a supportive, mission-driven team, you’ll educate families about Medicare benefits, coordinate admissions, and collaborate with physicians and case managers to ensure seamless care. This role emphasizes dignity, compassion, and patient-centered guidance.

Qualifications

• RN with a valid state license.
• Minimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred).
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.
• Active listener who builds trust and rapport.

Responsibilities

• Conduct comprehensive clinical assessments to determine hospice eligibility.
• Educate patients and families about hospice services and Medicare benefits.
• Collaborate with physicians, case managers, and interdisciplinary teams.
• Provide emotional support and clinical guidance during the decision‑making process.
• Initiate and complete the admission process, including documentation and coordination.
