VITAS Healthcare in Philadelphia is seeking a Hospice Admissions Nurse to bridge hospital or community care with the comfort of hospice. You will guide patients and families through one of life’s most important transitions with clarity, empathy, and confidence.

As an RN with a valid state license and at least 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred), you’ll conduct assessments, educate on Medicare benefits, and coordinate admissions.

• Initiate and complete the admission process, including documentation and coordination.
