Job summary

Live Well Homecare seeks a dedicated Hospice RN Admissions Nurse to perform in-home admission assessments and start-of-care visits. You will determine eligibility, educate families on hospice services, and tailor initial care plans to patient needs.

You will coordinate with physicians and the interdisciplinary team to support patients in remaining safely at home, applying strong clinical judgment and compassionate communication, with precise EMR documentation.

Qualifications

• RN license and active registration.
• Experience in hospice, palliative, or home health preferred.
• Proven clinical assessment and communication skills.
• Ability to work independently in a field-based setting.
• Patient-centered, compassionate attitude.
