Continuum-Care-of-Washington seeks an Admission RN to plan, organize and direct hospice care in patients' homes and communities. The role emphasizes nursing expertise and community health education.

The successful candidate will conduct initial and ongoing assessments, develop care plans, administer treatments, and collaborate with the clinical team to ensure compassionate, patient-centered care.

Qualifications

• Hospice nursing experience
• Community/home health care experience
• Ability to develop and revise care plans

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient/family as needs change.
• Initiate the plan of care and revise it as patient status changes.
• Use health data to determine nursing diagnoses.
• Develop a care plan with palliative actions and family participation.
• Administer medications and treatments as prescribed."
• Counsel patient and family in nursing needs and self-care techniques.
• Provide and maintain health care instructions as appropriate.
• Assist with activities of daily living and support self-sufficiency.
• Coordinate patient care as Case Manager when assigned.
