Job summary

Life's Journey Hospice is seeking an Administrator/Director of Clinical Services to lead nursing operations and oversee pre-admission evaluations. This role involves educating families, coordinating with intake and physicians, and ensuring quality end-of-life care at home or in care settings.

The ideal candidate is a licensed RN in Indiana with leadership experience, hospice training, and the ability to supervise a diverse team while managing crises and complex patient needs.

Qualifications

  • Graduate from an accredited school of nursing.
  • Currently licensed as a registered nurse to practice in Indiana.
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to crises outside of a hospital setting.
  • Adaptability to different social, racial, cultural and religious modes.
  • Completion of Hospice training program.
  • Minimum 2 years of RN experience, preferred.

Responsibilities

  • Performs pre-admission evaluations for hospice services and initiates admission processes.
  • Educates patient/family about services and obtains informed consents.
  • Coordinates with Intake Department, referring physician, and Hospice Medical Director.
  • Provides nursing care, emotional support and education to patients and families.
  • Maintains documentation and uses EMR systems as required.
  • Participates in weekend, holiday and on-call coverage.
  • Collaborates with IDT and other disciplines to coordinate care.

Life's Journey Hospice is committed to quality patient care, caregiver support and education. Our care allows terminally ill patients and their families to experience the end of life together, in the comfort and security of their home or wherever they call home.

We are looking for an Administrator / Director of Clinical Services to join our team! The Administrator / Director of Clinical Services performs pre-admission evaluations for appropriateness for Hospice services. Initiates the admission process by educating and informing patient/family of our services, obtains informed consents, provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life. This position also works with the intake team, referring physician and other referral sources to promote the hospice philosophy of care.

Essential Functions/Responsibilities:
  • Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.
  • Provides information, pre-assessment, and admission services to patients who have been referred to Life’s Journey Hospice; conducting pre-assessment visits in the home/Hospital or health care facility as necessary.
  • Communicates with the Intake Department, the referring physician and Hospice Medical Director to assure appropriateness of each referral and that necessary certifications are obtained.
  • Keeps the Intake Department informed about progress of referral/admission.
  • Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families. Encourages patients and family members to participate in the patient’s plan of care to meet established goals.
  • Utilizes computer skills to access medication information and document in electronic medical records as required.
  • Promotes teamwork by participating actively and appropriately as a member of an interdisciplinary team. Shares essential patient information with all members to coordinate care.
  • Collaborates with other members of the IDT in coordinating care to be efficient and financially responsible.
  • Makes appropriate referrals for other discipline services and medical equipment/supply companies to meet patient care needs in a timely manner. Coordinates care of patients with a variety of disciplines and community resources, both internal and external in a fiscally responsible manner.
  • Completes and submits documentation within agency timeliness policies that is complete, legible, and clearly reflects care given.
  • Provides hands-on nursing care for a diverse group of patients as assigned.
  • Utilizes appropriate resources to maintain knowledge and skills for the hospice care in all settings including regulatory/reimbursement guidelines and integrates into clinical practice.
  • Participates in weekend, holiday and on-call coverage as necessary.
  • Other duties as assigned by the Administrator/Director of Clinical Services.
Qualified applicants will possess the following:
  • Graduate from an accredited school of nursing
  • Currently licensed as a registered nurse to practice in the state.
  • Ability to work independently, make accurate, and at times, quick judgments.
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to crises outside of a hospital setting.
  • Acceptance of and adaptability to different social, racial, cultural and religious modes.
  • Completes Hospice training program.
  • Minimum 2 years of experience as a registered nurse, preferred. Active patient contact within the past three years, preferred.
