Job summary

Liberty Hospice is seeking an experienced Hospice Admissions Registered Nurse. The role requires an active RN license in the applicable state and involves evaluating and admitting patients in the home health setting.

You will develop individualized care plans, coordinate services, and document assessments. Full-time hours are Mon–Fri, 8a–5p, with on-call rotation and monthly staff meetings.

Qualifications

  • Current RN licensure in the state of the branch.
  • One year of clinical nursing experience required.
  • Graduate from an approved school of nursing.
  • Ability to perform home health evaluations, planning and coordination.

Responsibilities

  • Evaluate and admit patients according to LCD guidelines.
  • Initiate and develop an individualized plan of care for each patient.
  • Coordinate patient care services as necessary.
  • Document comprehensive assessment at point of care.
  • Work schedule per HR form; participate in on-call rotation and staff meetings.

Job Summary
  • Evaluate and admit patients as assigned according to Local Coverage Determinations (LCD) Guidelines provided by the branches Medicare Administrative Contractor (MAC).
  • Initiate and develop an individualized plan of care for each patient.
  • Coordinate patient care services as necessary.
  • Documents comprehensive assessment at point of care.
  • Works schedule according to Human Resource Action Form, participates in on‑call rotation, attends and participates in monthly staff meetings.
Job Requirements
  • Requires current RN licensure within state of branch location with one year of clinical nursing experience. Successfully completed nursing education from an approved school of nursing
  • Must be able to provide effective health care services to patients in their home setting through patient evaluation, care, planning, and coordination- to include thorough, accurate, and complete documentation of the initial patient visit.
  • Must accept travel and working conditions as assigned.
  • Must be able to work independently and possess strong communication, organization, and time management skills.
  • Must be able to use a personal computer and have a working knowledge of a variety of computer applications including Windows, Word, Excel, etc. CPR certification required.
  • Minimum of twelve hours continuing education annually.
  • Must have a valid driver's license.
