Guardian Hospice Care is seeking a Hospice Clinical Liaison to establish and maintain relationships with healthcare providers, hospitals, and referral sources, promoting hospice services and benefits.

Responsibilities

  • Building and maintaining relationships: Developing and nurturing relationships with healthcare providers, hospitals, nursing homes, assisted living facilities, and other referral sources to generate referrals for hospice services.
  • Educating healthcare professionals: Providing education and training to healthcare professionals about the benefits and services of hospice care, including pain and symptom management, emotional support, and end-of-life care.
  • Assessing patient eligibility: Evaluating patients referred for hospice care to determine their eligibility based on hospice admission criteria, such as a terminal illness with a life expectancy of six months or less.
  • Conducting patient assessments: Collaborating with the hospice interdisciplinary team to assess patients' physical, emotional, and psychosocial needs to develop an individualized care plan.
  • Coordinating care: Facilitating the coordination of care between the hospice organization, healthcare providers, and other healthcare professionals involved in the patient's care, ensuring a seamless transition from acute care to hospice care.
  • Providing support and guidance: Offering emotional support and guidance to patients and their families, addressing their concerns and answering questions about hospice care.
  • Documentation and reporting: Maintaining accurate and up-to-date documentation of patient interactions, referrals, and outcomes, and providing regular reports to the hospice organization on referral trends and outcomes.
  • Participating in marketing activities: Collaborating with the marketing team to develop and implement marketing strategies to promote hospice services, including attending healthcare conferences, seminars, and community events.
  • Staying updated on industry trends: Keeping abreast of the latest developments and trends in hospice care, healthcare regulations, and reimbursement policies to ensure compliance and provide accurate information to healthcare professionals.
  • Collaborating with the interdisciplinary team: Working closely with the hospice interdisciplinary team, including physicians, nurses, social workers, chaplains, and volunteers, to ensure the delivery of high-quality, patient-centered care.
