Job summary

AccentCare Inc. is seeking a Hospice Care Consultant in Rosemont, IL, to drive hospice sales and educate referral sources about our services. You will build relationships with physicians, hospitals, and community partners, ensuring timely, quality care for patients and families.

The role requires strong communication and independence, with a focus on ethical outreach, training, and collaboration with Admissions and Marketing to grow access to AccentCare’s hospice programs.

Qualifications

• Bachelor's degree or 5 years in home health or healthcare sales.
• Strong communication, problem-solving and relationship-building skills.
• Ability to work independently and manage a territory.
• Knowledge of hospice regulations preferred.

Responsibilities

• Develop and maintain trusting relationships with physicians, hospitals, facilities, and community orgs through in-person visits and education.
• Drive hospice sales by nurturing current relationships and identifying new referral opportunities.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate with Admissions and Marketing to develop community outreach aligned with our mission.
• Partner with clinical staff to ensure referrals transition smoothly into care.
• Participate in training and uphold confidentiality and ethical standards.

Pay: $70,000 - $95,000.00/year (based on experience) base salary+ monthly bonuses and mileage

As a Hospice Care Consultant, you'll be the face of AccentCare, building strong relationships with healthcare providers, facilities, and community partners. Your primary focus will be hospice sales, educating referral sources about our services, and ensuring patients receive timely, quality care.

Key Responsibilities

• Develop and maintain trusting relationships with physicians, hospitals, skilled nursing facilities, and community organizations through in-person visits and educational conversations, helping them understand when and how hospice care can best support patients and families.
• Drive hospice sales by nurturing current relationships, identifying new referral opportunities and increasing awareness of our programs.
• Conduct presentations and in-services to educate partners on hospice benefits and eligibility.
• Collaborate closely with Admissions and Marketing teams to thoughtfully develop and carry out community outreach plans that reflect the mission of honoring life and offering hope.
• Partner with clinical staff to ensure each referral transitions smoothly into care, always prioritizing patient comfort, dignity, and family support during this sensitive time.
• Uphold ongoing commitments to quality, ethics, and learning by participating in training, planning thoughtful outreach strategies, and honoring confidentiality in all aspects of your work
