The Hospice Admissions Nurse assesses patients in their home or in hospitals and prepares them for entry into the spectrum of services provided by Gilchrist Hospice Care. The role reports to the Admissions Team Manager and serves as an ambassador within the service area.
Requires a Diploma/ADN/BSN, current MD nursing license, and at least three years of nursing experience; home health or hospice experience is preferred. Competitive pay and comprehensive benefits are offered.

The Hospice Admissions Nurse assesses patients in their place of residence or in hospitals and prepares the patient for entry into the spectrum of services provided by Gilchrist Hospice Care. The Hospice Admissions Nurse serves also as ambassador for Gilchrist Hospice Care within the service area.
Diploma, ADN and/or BSN from an accredited institution
Current MD nursing license.
Three years of nursing. Home Health or Hospice experience preferred but not required.
Pay Range $32.18 - $49.46
Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
