Qualifications

Responsibilities

• Deliver personalized physical therapy services to home-based clients.
• Conduct initial and ongoing assessments to track progress.
• Perform fall risk assessments at Assisted Living Facilities (ALFs).
• Supervise client care to ensure best practices.
• Represent organization at ALF events with presentations.
• Travel to client locations and events as needed.
• Create and update care plans for non-medical clients.
• Handle administrative duties including calling PCP offices.
