Qualifications

• Bachelor’s degree in a health-related field required.
• Valid Tennessee Driver’s License and acceptable MVR.
• Criminal background check and automobile insurance required.

Responsibilities

• Conduct ongoing assessment and develop individualized service plans.
• Refer and link to community resources and advocate for consumers.
• Ensure consumers are seen upon discharge from hospital and Diversion Services within agency standards.
• Coordinate daily psychiatric, medical, and environmental needs.
• Attend and participate in team meetings and document plans.
• Develop and maintain community relations and PATH trainings.
