Qualifications

• Bachelor’s degree in Social Work or related field is required.
• One-year direct service experience, preferably in aging.
• Ability to maintain organized documentation and prepare reports.

Responsibilities

• Conducts needs assessments for individuals applying for case management.
• Advocates for disabled and older adults during the service delivery process.
• Coordinates activities to ensure cohesive service delivery.
