Help At Home is seeking a Registered Nurse (RN) for Home Visits in New York. The role focuses on coordinating patient care, education, and documentation to ensure high-quality home health services.

You will work with field staff to manage admissions, transfers, and visits, while adhering to regulatory standards and maintaining comprehensive clinical records. A NY RN license and CPR/First Aid certification are required, with at least one year of clinical nursing experience.

• Current CPR/First Aid certification.
• One year of clinical nursing experience.

Mansioni

• Monitor patient care plans and ensure compliance with home health standards.
• Coordinate patient admissions, discharges, transfers, and maintain medical records.
• Collaborate with field nurses and staff to assign cases and manage schedules.
• Educate patients and families on home health services and medications.
• Develop and implement quality control procedures for patient care.
• Review patient records with billing to resolve coding or insurance issues.
