Job summary

ViaQuest is seeking a behavioral health professional to provide scheduled patient visits and maintain current treatment plans for clients with diverse needs in a home/community setting.

Ideal candidates hold a Social Work license or equivalent, have therapy experience, and are willing to travel within the service area; a valid driver's license and reliable transportation are required. Opportunities include licensure supervision and full-time benefits.

Qualifications

• Bachelor’s degree in Social Work or a related field with one year of therapy experience or a Master’s degree in Social Work or Professional Counseling.
• Social Work/Professional Counseling License or equivalent: LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, LMFT, LIMFT, LIMFT-S.
• Experience in therapy and counseling is preferred.

Responsibilities

• Providing patient visits as scheduled.
• Ensuring treatment plans are up to date for each client.
• Completing comprehensive and succinct progress notes.
• Communicating and following up as needed regarding the client's status.
• Working collaboratively with those providing case management and/or psychiatric services.
