Job summary

Mozaic Senior Life is seeking a compassionate Speech-Language Pathologist (SLP) to provide skilled services in clients' homes as a per diem role in Connecticut. The SLP will evaluate, treat, and document communication and swallowing needs while collaborating with an interdisciplinary team and families.

The position requires a Master's degree in SLP, CT license, CCC-SLP eligibility, and at least 1 year of related clinical experience.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Minimum of 1 year clinical experience in long-term care, acute care, rehabilitation, home care, or related setting.
• Current Connecticut SLP license.
• ASHA CCC-SLP or eligibility.
• Valid driver's license and reliable transportation.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience with older adults and speech, language, cognitive, or swallowing disorders preferred.

Responsibilities

• Provide evaluations and treatments in clients' homes.
• Treat speech, language, cognitive-communication, voice, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Educate clients, families, caregivers, and healthcare professionals.
• Monitor progress and adjust treatment plans as needed.
• Communicate with physicians and the interdisciplinary team.
• Complete clinical documentation within established timeframes.
• Record billable treatment time per Home Care procedures.
• Demonstrate flexibility in scheduling.
• Maintain knowledge of evidence-based practices in Speech-Language Pathology.
• Provide compassionate, person-centered care to older adults.
