Qualifications

• One year of experience providing skilled care to the medically fragile.

Responsibilities

• Implements the medical plan of treatment and follows physicians' orders.
• Documents activities and services accurately and timely according to CHC and agency regulations.
• Monitors general health conditions and notifies supervisor of issues or need for treatment.
