Job summary

Plymouth Place Senior Living is seeking a compassionate Home Care Worker/Resident Assistant to support clients in a resident's home. You will assist with daily living activities, household tasks, and transportation, while monitoring health and wellbeing under a plan of care.

Role requires reliable transportation and, when applicable, state certifications. You will contribute to our Pillars of Excellence by delivering person-centered care and fostering independence in our residents.

Qualifications

• High school diploma or equivalent required.
• Previous experience in home care, caregiving, or related field preferred.
• Certification as a Personal Care Assistant (PCA), Homemaker, or CNA preferred if required by state policy.
• Valid driver’s license and reliable transportation preferred (if transportation duties are required).

Responsibilities

• Assist clients with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and mobility.
• Assist with household chores including cooking and meal prep, cleaning and laundry.
• Provide medication reminders as permitted by policy.
• Support clients with shopping, appointments, and transportation outside of the home as needed.
• Observe client condition and report changes to supervisor; document services provided.
• Provide companionship and emotional support while maintaining client independence.

• SALARY**
• $21/hour for a Resident Assistant working as a Home Care Worker in a resident's home.
• $18 - $19/hour for a Resident Assistant working in a level of living (Assisted Living, Memory Care or Skilled Nursing).
• $23/hour for a CNA working as a Home Care Worker in a resident's home.
• $20 - $22.50/hour for a CNA working in a level of living (Assisted Living, Memory Care or Skilled Nursing).

Personal Care Services

• Assist clients with activities of daily living (ADLs), including skin care, ambulation, bathing, grooming, dressing, and toileting, exercise, feeding, positioning as directed in the service plan.
• Assistance with household chores, including cooking and meal preparation, cleaning and laundry.
• Provide medication reminders as permitted by agency policy.
• Assistance in completing activities such as shopping and appointments outside of the home;
• Observation of clients functioning and reporting changes to their supervisor.
• Provide companionship and emotional support to clients.
• Assist with shopping, errands and transportation assigned.
• Support clients in maintaining independence and daily routines.
• Completion of appropriate records documenting service provision.

Safety and Client Support

• Maintain a safe environment and follow established safety procedures.
• Observe and report any safety hazards, changes in client condition, or concerns to the supervisor.
• Follow infection control practices and proper hygiene standards.
• Respect client rights, privacy, and confidentiality always.

Documentation and Compliance

• Document services provided according to agency policy and procedures.
• Follow the individualized care plan and agency guidelines.
• Maintain compliance with applicable state regulations, including standards established by the Illinois Department of Public Health (IDPH), when applicable.
• Attend required training and in-service education sessions.

Requirements

JOB REQUIREMENTS

ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS

The physical activities of this position constantly require effective talking, hearing, sitting, walking and standing functions. The position also often involves sedentary work where sitting is required most of the time, with occasional standing or walking. The position also often requires exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.

OSHA Exposure Category

Category III – Position includes tasks that involve no exposure to blood, body fluids, tissues; would not be required to perform Category I tasks.

INTELLECTUAL/EMOTIONAL REQUIREMENTS

The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure.
