Qualifications

• Must hold active state licensure or certification as applicable.
• Demonstrated professionalism, empathy and patient focus.
• Ability to work independently with strong communication.
• Comfort using mobile tech and secure messaging for care coordination.
• Willingness to travel to patient homes and manage supplies.
• Pass required background and driving checks.

Responsibilities

• Provide preventative care and diagnostics at patients' homes.
• Capture data such as vitals, ECGs, retinal imaging and samples.
• Coordinate with labs and care teams via secure apps.
• Maintain accurate patient records and ensure quality care.
• Deliver compassionate, patient-centered service in diverse homes.
