Job summary

AmeriPharma in Wisconsin is seeking a dedicated Registered Nurse (RN) with expertise in home infusion therapy to join our team. This role combines in-home care and patient education for treatments such as IVIG and IV biologics, under the supervision of the Director of Nursing.

The RN will conduct in-home visits, document clinical interventions, and serve as a resource for patients and their families while collaborating with pharmacy departments.

Qualifications

  • Graduate of an accredited nursing school with a current RN state license.
  • 2+ years of experience in home infusion and infusion centers.
  • 2+ years of experience with IV Immunoglobulins and IV Biologics; CRNI preferred.
  • Experience with peripheral IV starts, infusion therapy, and phlebotomy for adults and pediatrics.
  • Strong interpersonal, time management and organizational skills.
  • Excellent written and verbal communication skills.

Responsibilities

  • Conduct in-home infusion therapy visits with patients according to the plan of care.
  • Prepare clinical/nursing notes and document patient interventions in records.
  • Act as a clinical resource, collaborating with pharmacy departments.
  • Maintain knowledge of drug side effects and interactions.
  • Offer telephone and in-person patient education on therapy protocols and adherence.
  • Ensure patients understand therapy plans, medications, and adherence.
  • During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process.
  • Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.
  • Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs.
  • Manage patient and family expectations regarding services, outcomes, and discharge goals.
  • Establish primary and secondary diagnoses based on patient assessment and home health care focus.
  • Develop care plans with skilled interventions, medical supplies, and necessary services to meet goals.
  • Regularly evaluate patient progress, in collaboration with team members, and revise care plans as needed.
  • Perform other duties as assigned.
