Job summary

Enliven is seeking an experienced RN to service home infusion and infusion center patients in Windsor and surrounding locations. The ideal candidate will educate patients and families on prevention and healthy habits, and provide high-quality nursing care with little supervision. The role requires strong Med/Surg, ICU, and ER experience, IV placement skills, and current BLS.

ACLS/PALS are preferred. A flexible schedule and travel reimbursement are offered to support patient care across multiple

Qualifications

• Background check required.
• Malpractice insurance required.
• Drug screen required.
• Current CPR/BLS required.
• ACLS; PALS preferred.
• Home Health/Infusion experience preferred.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient and follow physician/Clinical Manager directions.
• Prepare and update medical documentation and care plans in the EMR system.
• Coordinate with physicians and pharmacies to develop treatment plans.
• Discharge patients after consultation and complete required documentation.
• Report patient progress and adjust care plans with the care team.
