Job summary

Hackensack Meridian Health At Home is seeking a Home Infusion Registered Nurse to deliver professional nursing care in patients’ homes. You will perform assessments, plan and implement infusion therapies, and collaborate with physicians to update care plans.

Qualifications

• RN license in New Jersey (NJ)
• Nurse with active RN license and education in nursing
• Experience with infusion therapy or home health nursing preferred

Responsibilities

• Provide professional nursing care to patients in home care settings.
• Demonstrate competency with infusion therapy and nursing procedures.
• Ensure orders are followed, document care in EMR, and coordinate with physicians.
• Maintain compliance with policies, payor authorizations and care plans.
• Participate in planning, teaching, and communication with the care team.
• Be on-call after hours as required and respond promptly to messages.
