Palomacares is seeking an experienced, dependable RN to provide skilled home infusion therapy in the San Angelo, TX area. The role travels to patients’ homes to deliver safe, high-quality infusion care per physician orders and agency policies.

The ideal candidate will administer IV meds, biologics, IVIG, and other therapies, perform assessments, manage IV access, and educate patients and caregivers. Part-time / PRN with flexible scheduling and local travel.

Qualifications

• Current, unrestricted Texas RN license.
• Current BLS/CPR certification.
• Minimum of 1 year of recent RN experience.
• Competency in peripheral IV insertion and central-line care.
• Ability to work independently in patients' homes.
• Reliable transportation and valid driver's license.
• Willingness to travel within the assigned territory.
• Smartphone and reliable internet access for scheduling and electronic documentation.
• Ability to meet applicable background, health-screening, and employment-compliance requirements.

Responsibilities

• Provide home infusion therapy, including IV medications, biologics, IVIG, antibiotics, hydration, and other prescribed therapies.
• Perform comprehensive patient assessments before, during, and after each infusion.
• Initiate and manage peripheral IV access and provide care for PICC lines, ports, and other vascular access devices.
• Monitor patients for adverse reactions and respond appropriately.
• Verify medication, dose, route, frequency, supplies, and orders before treatment.
• Educate patients and caregivers on medications, infusion procedures, vascular access care, and signs requiring medical attention.
• Communicate changes in patient condition to the pharmacy, physician, case manager, and clinical team as appropriate.
• Complete accurate and timely electronic clinical documentation following each visit.
• Maintain infection-control and medication-safety standards in the home environment.
• Travel throughout the San Angelo service area for scheduled patient visits.

Position Overview

We are seeking an experienced, dependable, and compassionate Registered Nurse (RN) to provide skilled home infusion therapy to patients throughout the San Angelo, Texas area.

In this role, the nurse will travel to patients' homes to deliver safe, high-quality infusion care in accordance with physician orders, pharmacy protocols, and agency policies.

Key Responsibilities

Infusion Therapy & Patient Care

• Provide home infusion therapy, including IV medications, biologics, IVIG, antibiotics, hydration, and other prescribed specialty therapies.
• Perform comprehensive patient assessments before, during, and after each infusion.
• Initiate and manage peripheral IV access and provide care for PICC lines, ports, and other vascular access devices within scope and competency.
• Monitor patients for adverse reactions and respond appropriately.
• Verify medication, dose, route, frequency, supplies, and physician/pharmacy orders before treatment.

Patient Education & Communication

• Educate patients and caregivers on medications, infusion procedures, vascular access care, and signs or symptoms requiring medical attention.
• Communicate changes in patient condition to the pharmacy, physician, case manager, and clinical team as appropriate.

Documentation, Safety & Compliance

• Complete accurate and timely electronic clinical documentation following each visit.
• Maintain infection-control and medication-safety standards in the home environment.
• Track supplies used and promptly report any supply or medication concerns.
• Travel throughout the San Angeloservice area for scheduled patient visits.

Required Qualifications
