Job summary

Enliven Home Infusion Specialists Arizona seeks an experienced Registered Nurse to service home infusion and infusion center patients in Show Low and surrounding locations. The ideal candidate will educate patients and families, provide high-quality nursing care, and handle emergent situations with composure.

Responsibilities include EMR documentation, patient visits, care plan implementation, and collaboration with physicians and pharmacists.

Qualifications

• Background check required.
• Malpractice insurance required.
• Drug screen required.
• RN with Associate or Bachelor Degree in Nursing.
• Minimum five years nursing experience in Acute Care (preferred).
• Strong Med/Surg, ICU, ER, acute care experience.
• Home Health/Infusion experience (not required, but preferred).
• Current CPR/BLS certifications required.
• ACLS; PALS preferred.
• Ports/Central lines/PICC line skills.
• Strong IV placement skills.
• Excellent organizational and communication skills.
• COVID-19 vaccination preferred by some partners/patients.

Responsibilities

• Complete daily electronic forms for each patient in EMR.
• Provide admission, case management, and follow-up skilled visits.
• Administer ongoing care/infusions and follow physician orders.
• Collaborate with physicians/pharmacies to develop treatment plans.
• Perform hands-on care and care plan evaluation.
• Report patient progress to physicians, pharmacy, and Clinical Manager.
• Implement care plans with patients and families.
• Discharge patients after physician/manager approval and document appropriately.
• Prepare medical documentation including care plans and updates.
• Update patient records and coordinate with healthcare teams.

Job description

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Show Low and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care.

Responsibilities: RN (Registered Nurse)

• Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System.

• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.

• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.

• Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.

• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.

• Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously.

• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.

• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.

• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.

• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.
