Job summary

Enliven Home Infusion Specialists Arizona is seeking an experienced Registered Nurse to service home infusion and infusion center patients in Maricopa, AZ, and surrounding areas. The RN will educate patients and families, perform skilled nursing visits, and collaborate with physicians and pharmacies to implement care plans.

The ideal candidate has at least five years of nursing experience in acute care, strong Med/Surg/ICU/ER skills, and current CPR/BLS. ACLS/PALS are a plus.

Qualifications

• Drug screen required.
• Associate or Bachelor Degree in Nursing required.
• Minimum of five years of nursing experience preferred in an Acute Care setting.
• Strong Med/Surg, ICU, ER, acute care experience.
• Home Health/Infusion experience preferred.
• Current CPR/BLS certifications required.
• ACLS and PALS preferred but not required.
• Skilled in accessing implanted ports, central lines, PICC lines.
• Strong IV placement skills.
• Strong organizational and communication skills.
• Vaccination against COVID-19 preferred by some partners.

Responsibilities

• Complete daily electronic forms for each assigned patient within an EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and provide follow-up as directed by physician and Clinical Manager.
• Collaborate with physicians and pharmacies to develop initial treatment plans.
• Deliver hands-on care and educate patients and families under physician guidance.
• Report patient progress to physician, pharmacy, and Clinical Manager.
• Implement care plans with patients/families to achieve outcomes.
• Discharge patients after physician and Clinical Manager consultation and finalize medical documentation.
• Prepare clinical documentation including case conferences, patient contacts, medication changes, and care plan updates.
• Prepare visit/shift reports and coordinate with other health care disciplines to optimize care.

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Maricopa, AZ, and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patient's well-being by providing high-quality nursing care.

Responsibilities:

RN (Registered Nurse)

• Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies in developing the initial plan of treatment based on the physician's orders and initial patient assessment.
• Provide hands‑on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
• Report patient care/conditions/progress to the patient's physician/pharmacy and Clinical Manager continuously.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other health care disciplines to provide optimum patient care.
