Job summary

Enliven is seeking an experienced RN to service home infusion and infusion center patients in Austin, TX and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision.

They will educate patients and their families on prevention and healthy habits, follow health and safety guidelines faithfully, and promote patient well-being by providing high-quality nursing care. Travel is involved to surrounding locations as needed.

Qualifications

• Background check required.
• Malpractice insurance required.
• Drug screen required.
• Associate or Bachelor Degree in Nursing minimum.
• Five years of nursing experience preferred in an Acute Care setting.
• Strong Med/Surg, ICU, ER, acute experience.
• Home Health/Infusion experience preferred.
• CPR/BLS required.
• ACLS and PALS preferred.
• Ports/Central Lines/PICC access.
• Strong IV placement skills.
• Excellent organizational and communication skills.
• COVID-19 vaccination preferred.

Responsibilities

• Complete daily electronic forms for each assigned patient in EMR.
• Provide admission, case management, and follow-up skilled visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and provide follow-up as directed by physician and Clinical Manager.
• Confer with physicians/pharmacists to develop initial treatment plan.
• Provide hands-on care and teach patient/family under physician supervision.
• Report patient progress to physician/pharmacy and Clinical Manager.
• Implement care plans with patient/family to resolve needs/problems.
• Discharge patients after consultation and complete clinical documentation.
• Prepare medical documentation including case conferences, patient contacts, and care plan changes.
• Prepare visit/shift reports and coordinate with other health care disciplines to ensure quality care.

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in AUSTIN, TX, and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patient's well-being by providing high-quality nursing care.

Responsibilities:

• Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies in developing the initial plan of treatment based on the physician's orders and initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
• Report patient care/conditions/progress to the patient's physician/pharmacy and Clinical Manager continuously.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other health care disciplines to provide optimum patient care.

Job Requirements:

• Background check required

• Associate or Bachelor Degree in Nursing Minimum of five years of nursing experience preferred in an Acute Care setting
• Strong Med/Surg, ICU, ER, acute experience
• Home Health/Infusion experience (not required, but preferred)
• Current CPR/Basic Life Support certifications (BLS) required
• Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (not required, but preferred)
• Skilled in accessing Implanted Ports; Central Lines; PICC lines
• Strong IV placement skills
• Strong organizational and communication skills
• Preferably COVID-19 Vaccinated (some pharmacy partners & patients require a vaccinated nurse)
