Job summary

Optum is seeking an experienced Home Infusion Nurse to deliver skilled nursing care in patients’ homes or ambulatory suites in southeast Ohio. The role offers autonomy, a flexible schedule, and meaningful patient connections.

You will join a multidisciplinary team with comprehensive training and support to ensure safe, effective infusion therapies. A valid RN license in Ohio and infusion experience are required.

Qualifications

  • Current, unrestricted RN license in Ohio or a valid Multistate RN License via NLC with Ohio authority.
  • Current BLS/CPR certification or willingness to obtain before start.
  • 1+ year RN experience in infusion/IV therapy (ICU, Med Surg, ED, PACU).
  • Ability to work independently with strong clinical judgment and collaboration skills.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Travel to patient homes/sites using a personal or rental vehicle per travel policies.
  • Provide safe, compliant infusion services per policies and physician orders.
  • Educate patients/caregivers on self-care and self-administration.
  • Assess home infusion suitability including safety and caregiver support.
  • Maintain patient histories, assessments, documentation, and plans of care.

As a Home Infusion Nurse, you'll provide one-on-one, skilled clinical care to patients where they feel most comfortable-at home or in an ambulatory infusion suite. This role offers true autonomy, a flexible schedule, and the opportunity to build meaningful connections with patients and caregivers while delivering life-changing therapies.

You'll have the support of a multidisciplinary care team, comprehensive training, and the resources you need to deliver safe, effective, and compassionate infusion care.

In This Role, You Will:

  • Work 30-35 hours per week
  • Travel within the southeastern Ohio area
Primary Responsibilities:
  • Travel to patient homes and related sites using a personal or rental vehicle in accordance with company travel policies
  • Provide safe and compliant infusion services in accordance with Optum Infusion Pharmacy policies, protocols, and physician orders
  • Establish and maintain positive therapeutic relationships with patients and caregivers, including education on self-care and self-administration
  • Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient independence
  • Obtain and maintain accurate patient medical histories, clinical assessments, and all required documentation, including plans of care, visit notes, medication profiles, and discharge summaries
  • Deliver skilled nursing care across a range of patient ages and conditions based on physician orders and treatment plans
  • Establish, access, and maintain vascular devices, including peripheral IVs, PICC lines, tunneled catheters, and implanted ports
  • Administer medications and monitor patient responses, including managing infusion reactions, vital signs, side effects, and lab collection
  • Communicate patient status and therapy response promptly with physicians, pharmacists, supervisors, and other care team members
  • Assist in staff orientation and training, serving as a clinical educator and resource as requested

Required Qualifications:
  • Current, unrestricted Registered Nurse (RN) license in Ohio or a valid Multistate RN License issued through the Nurse Licensure Compact (NLC) with the ability to practice in Ohio.
  • Current BLS/CPR certification or willingness to obtain prior to start.
  • 1+ years of RN experience in a clinical setting supporting infusion or IV therapy (e.g., ICU, Med Surg, ED, PACU).
  • Ability to work independently with strong clinical judgment and effective interdisciplinary collaboration skills.
  • Valid driver's license and reliable transportation
Preferred Qualifications:
  • Previous home infusion experience
  • Pediatric experience or open to training for pediatric infusions
  • Familiarity with Microsoft Office and EMR platforms
  • Knowledge of Infusion Nursing Society (INS) standards
  • Understanding of HIPAA, OSHA, and home infusion accreditation guidelines
  • Proven solid communication, organization, critical thinking, and teaching skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
