Job summary

Avera at Home Hospice-Sioux Falls is seeking a Registered Nurse for home hospice care in the Sioux Falls area. The role involves nursing care in patients' homes within a 40-mile radius of Sioux Falls, with on-call requirements about twice monthly.

You’ll work within a multidisciplinary team, following ANA standards, and contribute to moving health forward for patients and families. A sign-on bonus may be available.

Qualifications

• Active RN license in the state of practice.
• Minimum two years related experience preferred.
• Valid driver’s license and ability to be insured.

Responsibilities

• Collects patient health data.
• Analyzes assessment data to determine nursing diagnoses and patient focus.
• Identifies patient-specific outcomes and goals.
• Develops a care plan with prescribed interventions.
• Implements interventions in the plan of care.
• Evaluates patient progress toward outcomes.
• Evaluates quality and effectiveness of nursing practice.
• Collaborates with patient, family, and healthcare providers.
• Uses evidence-based findings where appropriate.
