Job summary

Hospice of Central PA is seeking a Home Hospice Visit Support RN for per diem in Harrisburg. You will travel to patients in the community, collaborate with the IDT, and provide nursing services and care coordination.

This role offers very flexible hours with a new orientation program; some training can be done from home. You must meet PRN requirements: minimum 24 hours per month and two Saturdays per quarter, PA RN license required.

Qualifications

• Registered Nurse, licensed in Pennsylvania.
• At least 6 months of recent nursing experience preferred.
• Must be vaccinated for influenza or have a religious/medical exemption.

Responsibilities

• Works in collaboration with the Interdisciplinary Team (IDT) providing nursing services, assisting with care coordination, and assessing and addressing the ongoing needs of patients and caregivers.
• This role is out in the community visiting patients in various settings.
